At NCHC, our Care Team is family centered and family focused. We recognize that the demands and responsibilities of busy life can be challenging and providing the desired level of supervision and attention to your loved one can prove to be difficult. Sometimes a helping hand is needed with everyday tasks that have become tricky for an aging person. We have been there and understand how difficult it can be to reassure that your loved one is cared for whenever and wherever they need it, especially when family or friends are unable to be present. We have decades of valuable experience among our Care Team. We have cared for our parents, grandparents and loved ones, and we always have the utmost respect and compassion for those whom we give care.

INITIAL CALL AND MEETING

Throughout your initial call, we ask a series of questions to form a basic needs assessment. Next, we set up an in-home visit with you and your loved one. The visit allows us to gather more information, answer questions, and gives an understanding of how we can help.

CUSTOM CARE PLAN

During your initial consultation and in-home needs assessment, we immediately begin to examine your needs and prepare a custom Care Plan for your loved one, matching your loved one with the right caregiver to implement the plan. With NEXT CHAPTER HOMECARE services, you will always know what services are being provided to your loved one. We then arrange a follow-up meeting with you to review the custom Care Plan and modify it as you see fit.

CAREGIVERS

Once you approve the custom Care Plan, we begin our examination to determine the most appropriate caregiver to implement care. Our approach to caregiver matching takes several factors into consideration, including services needed, your loved one’s interests, personalities of your loved one and the caregiver, and the overall situation. All our NCHC caregivers go through a comprehensive background check, including criminal and DMV check, an extensive interview process, and are fully insured and bonded. The care you receive from our NCHC Care Team feels like family because we are a family.
